This notice describes how medical information about you may be used and disclosed and how you can get access to the information. Please review it carefully.
Under recently enacted federal regulations, Woodhull Rx Center and all similar health care providers are required and obliged to maintain the privacy of your protected health information (PHI).
PHI is information, which may identify you and would relate to your health, your conditions and health care related services whether physical or mental.This notice describes how we may use and disclose PHI information to third parties for purposes of:

1. Dispensing Rx(s) pharmacist may have to contact other  physicians seen by you    or to your physician via mail, phone, fax,   e-mail or directly about your Rx(s).
2. For payment purpose, pharmacy staff   may have to contact your  Insurance   Company or Pharmacy Benefit Manager   utilizing PHI  information.
3. For health care related operations to   improve quality and  performance of our   pharmacy relating to your care disclosure   may be required to Secretary of Health   and Human Services.

Woodhull Rx Center is prohibited from disclosing your PHI unless state or federal law requires such disclosure, or other exceptions including law enforcement procedure without your consent. We may use or disclose your PHI in accordance with the Privacy Rules without obtaining your consent or authorizations
in the following instances: 

1. Business associates rendering services for us under contract in which your PHI information must be disclosed in order to perform their job under contracted conditions. For example – billing agency-providing billing for services rendered by this pharmacy. Business associates are required to safeguard PHI within their business associate contract with this pharmacy.

2. Discussions with individuals involved in your care or in the payment for your care if deemed appropriate by the health care professional.

3. Communicate to you of health related benefits or services which may provide (examples) –Refill reminder services, alternative treatments and options.

4. Food and Drug Administration if deemed necessary and important in lessening or preventing impending or serious health or safety threats to you or the public.

5. To comply with laws relating to workers compensation claims. 

6. Public health purposes as required by law to public health or legal  authorities.

7. As required by law.

8. Health oversight authorized by Law Including audits, i.e. investigations and inspections required for Woodhull Rx Center licensure, government monitoring of health care programs, and  civil right issues.
We are permitted to use and disclose PHI in accordance with the Privacy Rules for the following purposes.

1. Coroners, Medical Directors, Funeral Directors in accordance with law applicable to carry out their duties and functions.

2. Organ or Tissue Procurement Organizations in accordance with applicable law.

3. Research when approved by an Institutional Review Board (IRB) under established protocols ensuring privacy of your information.

4. Judicial or administrative Proceedings.

5. Correctional Institutions if you become an inmate.

6. Law enforcement purposes: court orders, Board of Pharmacy.

7. Assistance in or Notification of Family member, someone  responsible for your care or your personal representative of your  location and condition.

8. Victims of abuse, neglect, and domestic violence to government authorities, protective service agencies if we reasonably believe you are a victim of above. Such PHI will be disclosed if permitted or required by law, if you agree to such disclosure, necessary to prevent serious harm to you or someone else, or to official requesting such information represents it is necessary and will not be used against you (example) Notification by official would be in writing- An attorney must show attempt to reach person whose records are being subpoenaed. <The person whose records are being subpoenaed has a right to stop such release>.Military Authorities: if you are a member of the armed forces, we may disclose your PHI under military command authorities.National Security and Intelligence Activities – to authorized federal officials  as authorized by law.

10. National Security and Intelligence Activities – to authorized  federal officials as authorized by law.

Other Uses and Disclosures:

Woodhull Rx Center shall obtain your written authorization before using or disclosing your PHI for purposes other than those listed above or otherwise permitted or required by law. You may revoke such written authorization in writing and upon such notice,

Woodhull Rx Center shall stop using or disclosing previously authorized use or disclosure information. The revocation of your prior authorization shall be effective for all future uses and disclosures.

Request for Restrictions of PHI use and Disclosure for treatment, payment, health care operations:
You have the right to request restriction of certain uses and disclosures of PHI purposes of treatment, payment, HCO to family members, relative, representative or close family friend; however, we are not required by federal law to comply. If you request a copy of your PHI, you also have the right to request it be sent to a different location and by alternate means.

Complaints relating to a violation of your privacy rights may be filed with the Privacy Officer or the Secretary of Health and Human Services. No retaliation will be incurred for filing such a complaint.
